Probably a dumb question
but... in order to get a state flag from a state visit, does that visitor need State flags counter on their site?

I don't have a state flag counter and have all 50 states plus DC

US states and Canadian provinces are collected for free on the "normal" flag counter. So, you don't need necessarily a separate one.

on my country flag collector it does not show the states, just one flag for USA, am I doing something wrong?

On my "details" page (after I click on "view all" on the first page), it lists all the countries and just shows one flag for U.S., but if I click on the "plus" symbol to the left of the U.S. flag I get a drop-down with all the state flags that I've accrued.

You have actually collected 49 US states. That's what your flag counter is saying, not the US counter underneath the world map.
