Unknown hits
Can someone explain why there are many "Unknown" hits?
For example North Macedonia, 7 out of 10 hits will come as "Unknown" but it can happen for many countries (Sudan comes to mind), while some countries have pretty low number of unknowns.
Are regional IP addresses not recognized as such by the database or where's the problem?

Those are IPs that the providers never assigned regional data to. In Macedonia's case, though, most of them are Skopje, which has no ISO code
251/251 countries and 3,860/3,860 flags collected.

Is that based on evidence (you work for a (national) provider, know the nr. of providers/(assigned) IP blocks)?
Because if they never assigned regional data to IPs, I assume you can only speculate it's from the capital (which is weird they didn't assign IP blocks to the capital), but can't really see or check it?
How would you explain that some regions stopped working 10 months ago?
It makes me unsure about the accuracy of the counter and the decision to prolong it.

Skopje does have a city assigned, but not the region, since the region has no ISO. And as for the regions that died 10 months ago, the providers reassigned them to different ones or simply removed the geolocation data from the IPs. It's that simple. FC is based on Maxmind's GeoIP2 database that updates twice a week so the data changes VERY often
