visitors not showing on counter - answers to your questions
This is from yesterday. The reason I know they were there is that they left comments on photos. Now the lady from Australia has been back twice...and nothing has shown up yet.

Looks like there is no Flag Counter on those pages, maybe it's just on your profile?
For us to see the visitor, the visitor has to see your Flag Counter.

All I see here (and not on that link you posted) are two US states counters. How do you expect a World flag to show up on a US State counter?

What's next: how does one get the counter on each photo? 
Set up a website and put 23243 counters for all I care, but I seriously doubt flickr has that option, you should contact them

You don't have to do that. Just get your World Flag Counter here, copy the code and paste on your Flickr Profile area, as I said here
It doesn't matter if someone visit your photos or albums only, it is counted. Just make it sure it's a World Flag counter and no The US states counter

If the counter is only on the user's profile, it won't count any visits to picture pages. A user has to load the Flag Counter for us to see their visit.

I made this on my own flickr account and I think it worked....I'm going to chek that again. Otherwise, we can't put the FC on every photo or album, just on our profile.
