Germany is called "Nimetchina" in Ukraine
Hungary is also called "Ugorshchina" in Ukraine

Yes, I guess Germany is probably the most altered name over different languages..

I think the most "altered" is Ivory Coast, because in all languages they translate "ivory" and "coast":

In Vietnamese the names of several countries are written in very uncommon ways. Some examples:
Áo = Austria
Ba Lan = Poland
Bỉ = Belgium
Bồ Đào Nha = Portugal
Đan Mạch = Denmark
Đức = Germany
Hy Lạp = Greece
Nga = Russia
Pháp = France
Tây Ban Nha = Spain
Thổ Nhĩ Kỳ = Turkey
Thụy Điển = Sweden
Ý = Italy
